Introduction to Human Factors
This session introduces participants to some of the core principles of human factors. It explains why things work, and why sometimes they don’t. It details aspects of cognition and decision making, and relates them to everyday life and clinical settings. The session then focuses on how this understanding can be used to shape the way we live and work, enabling us to design for success, rather than requiring humans to be perfect. Creating Outcomes: Action Design and Implementation Masterclass
This masterclass helps participants understand why some safety actions are more effective and sustainable than others, and how different action designs will affect system performance and risk control. Whether the action is in response to an issue that has occurred, or a service innovation, this session will give you the knowledge and understanding to minimise your risks and maximise your benefits. Compassionate Engagement with Patients, Families, and Staff following Patient Safety Events

PSIRF Fundamentals
This session covers the fundamentals of PSIRF in one day. Participants will leave understanding core concepts of human factors and systems thinking, as well as just culture and restorative practice. Participants will learn learning-response methods such as ‘SWARMS’ and ‘After Action Reviews’ to capture learning from events, and understand how these come together to underpin the PSIRF approach to improve patient safety and experience. PSIRF Investigation MasTerclass: Three Days
This programme develops participants' knowledge and understanding of investigation and systems-based learning to a greater level, focusing on different tools used within safety-critical industries that are relevant to patient safety, but not covered by other training. This takes your toolbox to the next level and equips you to design for safety and performance, not just address safety and performance issues once identified. Thematic Analysis
This course provides a practical deep dive into thematic analysis, tailored to support your organisational priorities in PSIRF planning, incident learning, and evaluation. Participants will develop the skills to define scope, identify relevant sources of safety intelligence, generate meaningful categories and themes, and apply structured, system-focused methods consistently in practice. Using realistic scenarios, the workshop builds both competence and confidence, enabling participants to turn data and investigation findings into clearer organisational learning and improvement.

PSIRF- Systems Approach to learning from patient safety events (2-day)
This session further develops participants knowledge and skills in applying human factors methodologies to learning from safety events. This meets the standards mandated for Learning Response Leads within the NHS syllabus to enable staff to lead investigations within their organisation. It provides them with the skills and knowledge to apply a systems approach to learning from events, and how to compassionately and effectively engaged with this involved in and effected by patient safety events.
